Back to Nebraska by popular demand, national expert and healthcare consultant, Tim McNeill, RN, MPH, will facilitate an action-oriented workshop for NALHD and key partners on January 5, 2018.

Mr. McNeill presented models and partnerships to increase Medicaid/Medicare Reimbursement for community based public health services at the June 2017 Chronic Disease Summit (in Omaha). He specializes in health program development and sustainability and has been the lead technical assistance provider to establish integrated networks of LTSS providers in 26 states. Mr. McNeill has started or expanded multiple sustainable health programs including community-based free clinics, Federally Qualified Health Centers, two Medicare Shared Savings Program Accountable Care Organizations (ACOs), and LTSS networks. Mr. McNeill is a Registered Nurse with a Bachelor degree from Howard University and a Masters of Public Health from Eastern Virginia Medical School. Mr. McNeill is also a U.S. Navy Nurse Corps Veteran.

· Social Determinants of Health primer
· Background on MACRA incentives, CPC+ (Comprehensive Primary Care Plus), and opportunities for community collaborations
· Regional group breakouts—LHD Directors will have time with their partners to explore opportunities for collaborations and will receive feedback from Mr. McNeill
